Service for High-Risk Eating Disorders (SHED)

 
The Service for High-Risk Eating Disorders (SHED) was established following the release of the ‘Eating Disorders: A Framework for Wales’ document by the Welsh Assembly Government in 2009. 
 
SHED is a Tier 3 eating disorders service covering both Cardiff and the Vale UHB and Cwm Taf UHB. The main clinical base for the service is within the area of Cardiff and the Vale of Glamorgan, with an additional administrative base provided within Cwm Taf to support the provision of our clinical service within that Local Health Board area.
 

The Team

 
The SHED team's Clinical Lead is Dr Menna Jones.  She is supported by a Specialist Clinician, a Dietician, an Occupational Therapist, a graduate Mental Health Worker, a Team Administrator and a Consultant Psychiatrist.

Entry and Exit Criteria

 
SHED works with clients in the community who are over 18 and are experiencing a severe eating disorder .
 
Clients are referred to the service if their BMI is less than 15 (less than 16 for men), or there is evidence of high physical risk due to the eating disorder. In addition, if they show a high level of complexity within the eating disorder, or a complex interaction between the eating disorder and another mental health problem, they will be referred to SHED.
 
When clients consistently no longer meet the entry criteria, they will be deemed as no longer requiring the services of SHED and will be discharged to Tier 2 services, namely Eating Disorders Specialist Outpatient Treatment (EDSOT) and Community Mental Health Teams (CMHTs).

Services Provided

 
The Service for High Risk Eating Disorders provides 4 main services: 
  1. Highly specialised interventions for clients and their supporters including:
    • Dietetic interventions
    • Occupational Therapy Interventions
    • Psychological Therapies
    • Psycho-educational and contemplative-based therapeutic activity
    • Family support & education.
  1. Liaison with Tier 4 (inpatient) services by acting as gatekeeper and providing input into the care plans of patients.
  1. Consultation with Tier 2 (EDSOT and CMHTs) services to enhance confidence and competence working with people with severe eating disorders.
  1. Specialised assessments which evaluate the need for immediate care and inform the design of the SHED treatment package around the needs and wishes of the client.
Participation of Service Users and Their Personal Networks
 
The SHED team will actively promote the participation of service users and their personal networks within the service, and in particular in the following areas:
  • Participation in the development and review of their individual packages of service provision.
  • Collaboration of clients and their personal networks with SHED staff in guiding service development within SHED and the wider eating disorders service.
  • Evaluation of the SHED service.
  • Collaboration with B-eat Cymru to promote the participation of service users and their networks locally and nationally.
