Ward Doctor
The ward doctor is based on the ward from Monday to Friday 9.00am - 5.00pm. During the evening (5.00pm -10.00pm) there is an on-call doctor based in West Wing. Out of hours medical cover is provided by the GP out of hours service.
Ward Pharmacist
The Pharmacist visits the Ward weekly to review medicines.
Physiotherapist
On admission you will be assessed by a physiotherapist. A personal regime will be devised to suit your personal needs and abilities. Physiotherapy is available Monday - Friday on an individual basis and in groups. Regular physiotherapy groups are held which include Tai Chi, Yoga, Boccia and multisensory groups.
Occupational Therapist
Therapy is available from 08:30 – 16:30 Monday to Friday excluding Bank Holidays. You will be assessed by the OT on admission if your are requiring rehabilitation . The OT will assist you in preparing for your discharge home. Regular occupation therapy groups are held including reminiscence, flower arranging and cookery groups.
Psychologist
If a patient needs to be seen by a Psychologist, a member of the Multi-Disciplinary Team will make a referral and a Psychologist will attend from another hospital.
Nurses
During patients admission to Rhydlafar Unit they are allocated to one of 2 teams and a Nurse who will coordinate their rehabilitation programme. She will be responsible for planning and implementing all nursing needs during patient stay. This nurse is a patients ‘Named Nurse’
Speech And Language Therapist
If a patient has difficulty in communicating and / or swallowing food or drink, they can be referred to a Speech and Language Therapist by any member of the Multi-Disciplinary Team.
Podiatry
A Podiatry Service is available if required.
Dietitian
Patients' nutritional status will be assessed on admission and will be referred to a dietitian if there are any concerns. A dietitian is available for advice.
Social Worker
If a patient already has a Social Worker when transferred to Rhydlafar Unit, contact will be made by the Nursing Staff. If the patient requests a Social Worker, a referral will be made by the patient's Named Nurse. The Social Worker may attend a Multi-Disciplinary Meeting to discuss issues in preparation for patient safe discharge whether this is to patient's own home or Residential / Nursing Home.
Dentist
Patient may need to be assessed by a dentist during their stay - they will advise on any treatment required.
Ward Coordinator
Ward Coordinators are often the first point of contact for both staff and visitors to the ward. They welcome patients, visitors and relatives and ensure tasks are carried out and messages taken for other staff members. They play an integral role in the daily running of the ward and are responsible for all administrative tasks including answering telephone queries and maintaining accurate and up to date records.
