Welcome to Reablement Services


We are a team of healthcare professionals team funded by both Health and Social Services to ensure a safe, timely discharge from hospitals by intensive multi-disciplinary (Therapy and Carer) input in the home evironment, and we also help patients regain independence following safe discharge from hospital.

 
We offer two services:

  • Full team input (from relevant professions listed below and Reablement Carers)
  • Therapy only input (from one or more of professions listed below) 

 
The team has input from:

  • Physiotherapy
  • Occupational Therapy
  • Speech and Language Therapy
  • Dietetics
  • Podiatry
  • Social Service Carers

What is Reablement and how does it work?

  • The Reablement Team is Multidisciplinary - that is, different professionals including therapists and carers provide an intensive, home-based self-help and therapy programme upon safe discharge from hospital, to help you regain and maintain independence.
  • The staff on your ward will discuss Reablement with you and, if appropriate, complete the referral to the Reablement Team, identifying the services you will benefit from once you're discharged.
  • Staff from the Reablement Team will visit you on the ward before discharge to discuss Reablement and assess your needs once discharged.
  • Reablement staff will visit you at home as soon after your discharge as possible, and any problems/concerns encountered will be recorded/resolved.
  • A personal reablement programme will be discussed, agreed and prepared, based upon the team's assessment of what will benefit you the most.  This programme will describe the help you need, as well as making clear what you hope to be able to do yourself.
  • The programme and your progress will be regularly monitored.
  • As you become more independent, the level of support supplied by the team is gradually reduced.
 

The Reablement Team comprises of a range of professionals, such as:



Registered Home Care Manager and Home Care Coordinator
Organises your Home Care programme with specially trained carers who will help you to do as much as possible for yourself (Involved in Full Team only).

Occupational Therapist (and the O.T. Technicians)
Will advise the Reablement Carers and you on the best way to help without taking away your independence.  They will also advise on any equipment or techniques to improve your independence.

Physiotherapist (and the Physiotherapy Technicians)
Will help you to improve your mobility and strength.  This may include advising you on the right walking aids and setting an exercise programme for you.

Speech and Language Therapist
Will assess your communication needs and will provide therapy and advice as appropriate.  They will also help if you are having any problems swallowing.

Dietician (where indicated)
Will work with you and other members of the Team if you have any concerns about your nutritional well-being.

Podiatrist (where indicated)
Will provide treatment to aid your rehabilitation and maintain mobility.  They will advise you on foot care and footwear.
 
 
 

How long does the service last?

 
Everybody’s progress is different but we aim to achieve the objectives within approximately six weeks.  Before your programme ends, there will be a review to discuss any longer-term needs.
