The letter we send to you will give details of the time and date of your appointment, where to report and what to do when you arrive. If you cannot keep your appointment for any reason, it is important that you let us know immediately. Please telephone or write to the relevant hospital / clinic address so we can then offer the date to someone else, and arrange a more convenient appointment for you. Failure to do this could result in your name being taken off the waiting list, and you being referred back to your General Practitioner.
Our aim is to achieve well-run Outpatient Departments. This can only work with your help and co-operation, especially in reducing waiting times. Please try to arrive approximately 10 minutes before your appointment time. Patients are seen the order of their appointment time, so there is no benefit in arriving early. In fact, this is likely to increase the time you have to wait.
- your hospital letter or card
- any correspondence from your family doctor
- any specimens you have been asked to bring (if you need to bring them in a container, please make sure it is thoroughly washed and clean before you use it)
- your medication card and / or any medication you are currently taking
- your current allowance book if you are receiving Income Support or Family Credit
- your glasses if you are attending an eye clinic
- a list of questions you may like to ask the doctor or nurse
- small change for refreshments or car parking
- a book or magazine to read if you are on your own
- any changes in personal details, such as address or phone number.
