Pre-Dialysis Service
There are six Pre-Dialysis Nurse Specialists, each working with two Nephrology Consultants in geographical areas.
As a key part of the Multi Disciplinary Team (MDT), we aim to provide timely and comprehensive pre-dialysis education, expert advice and support to individuals (and their families / significant others) with chronic kidney disease, by means of home visits, telephone contact and nurse-led clinics. We aim to ensure a smooth transition to the chosen treatment.
As a team, we strive to be professional role models and effective patient advocates.
In addition, through collaborative working with other professionals and organisations we aim to support, educate, encourage and empower patients and other members of the MDT.
Each Clinical Nurse Specialist has their own area of expertise. These roles include:
- a Pre-Dialysis Specialist Nurse who assesses complex patients and liaises closely with the Pre-Dialysis team, aiding smooth transition to chosen therapy (Jean Jenkins)
- an Access Co-ordinator who organises and prioritises theatre slots for dialysis access (Ann Jones)
- a Pre-Dialysis Anaemia Co-ordinator who oversees the renal Anaemia service, conducting regular audits of the service (Justine Aggett)
- a Specialist Nurse who manages the late presenters to the service (Nerys Thomas)
- Specialist Nurses who manage the Conservative Management Programme (Julia Pugh and Alison Prichard).
