Our aim is to provide the best quality holistic multidisciplinary team rehabiltation service to patients, in order to maximise their functional independence and psychological well-being in the overall context of 'Chronic Disease Management'.

What we do

John Pathy Day Hospital plays a key role in supporting patients to optimise their recovery and maintain their indepence in the community by providing a full multi-disciplinary assessment and treatment.

We play a significant part in expediting discharges from hospital and avoiding hospital admissions.

We have access to basic diagnostic services.

Philosophy

As a Multidisciplinary Team our philosophy is to provide the best Rehabilition Service to all our patients,using a holistic approach to the care that we give. In order to fulfil this goal we aspire to:-

  • attain the highest standard of performance possible to meet patient's individual needs
  • provide a clinically effective service with evidence based practice treating patients with courtesy and respect
  • provide an equitable service, regardless of ethnic origin, religious beliefs, sexuality or disability
  • provide a friendly, supportive atmosphere respecting patients' privacy and confidentiality; maximising independence specific to individualised needs within their abilities
  • ensure that our staff are a dedicated multidisciplinary team who promote educational resource for patients by maintaining and updating their knowledge and skills
  • acheive the ultimate goal of retaining paitients safely in the community and avoid unnessary hospital admissions.

  • Falls Management Programmes
  • Continence Clinic


Referrals can be made by Health Care Professionals only. Patients must require input of at least 2 members of the multi-disciplinary team
  
Day Hospital Structure

  • 40 day places Monday - Friday
  • Male and female places
  • Ambulance transport support
  • 65 years of age and over
