In order to ensure that all centres comply with regulations we are required to complete registration, consent and outcome forms for all licensed treatments undertaken within the Unit.
The consent forms are required to ensure that all centres maintain accurate records of licensed treatments undertaken and to ensure compliance. These forms are mandatory for all patients attending the Unit.
