Aim of Elizabeth Ward
The ward aims to assist the patient to achieve his/her maximum potential - ensuring that he/she reaches their highest level of independence and that recovery takes place as quickly as possible. Our teams assist patients to maintain their strength and give them a feeling of well-being. Elizabeth Ward aims to ensure a safe discharge, whether it is to the patient's own home, residential or nursing home. Patients who have ongoing needs are given the highest standard of care that we can achieve and encouragement is given so that carers are actively involved, as much as they/the patient wishes them to be, in all aspects of care planning.
What we do
The Elizabeth Ward Team cares for older people who require input with a multi-disciplinary approach. Our efforts are directed at improving the person's health so that he/she is then able to achieve his/her maximum ability in all aspects of daily living. Whilst this care is being offered by the multi-disciplinary team, we seek to achieve the best outcomes for the person in his/her choice of future accommodation. Patients range in needs from those who are very ill to those who are ready for the final stages of discharge planning.
