Welcome to the District Nursing Service
The District Nursing service is part of Cardiff and Vale Health Board. District Nurses are employed by the UHB but work very closely with GP’s and other health and social care professionals within the local community. They also have a close working relationship with the voluntary sector.
The aim of the service is to provide skilled nursing care to patients in their own homes, by specially trained community nurses, and to provide support to carers. The service is provided over the 24 hour period, 365 days a year, and is available currently to all individuals over the age of 16 years of age.
We predominantly offer home visiting to patients who are housebound, or unfit to travel due to illness. Treatment room appointments are available for those patients who are mobile and require District Nursing care rather than care from the Practice Nurse with the GP surgery.
The District Nursing service also has specialist links to the Acute Response Team, which provides an early hospital discharge scheme or prevention of admission service. Specialist Wound Care and Continence Services are also available.
What we Offer For Patients
- Assessment of nursing need, and a prescription of nursing care.
- Varying clinical interventions - care for the terminally ill, wound care, continence services intravenous, enteral and oral medication provision.
- Provision of specialist community equipment.
Locations
District Nurses are based throughout the community of Cardiff and Vale, some in community clinics or with GP premises.
