Welcome to the Continuing Care Support and Sitting Service
The Continuing Care Support and Sitting Service is a community based provision intended at providing day or night support for patients and carers who live in the geographical area of Cardiff and the Vale of Glamorgan, and who are registered with a GP in the specified area.
The Patient Support and Sitting Service is a part of the Continuing Care Service. Therefore, patients accessing the service will have predominantly been assessed and deemed eligible for Continuing NHS Healthcare.
The eligibility criteria for this service fall into the following priority order:
- meet eligibility criteria for Continuing NHS Healthcare
- terminally ill
- life limiting chronic illness.
Referrals
All patients should be referred to the sitting service via the District Nursing Service. It is the responsibility of the District Nurse Team Leader to assess patients for suitability for sitters.
What The Sitting Service Can Offer
The service is restricted in availability, and access to the service is dependent on demand and availability of staff. We aim to provide support for up to 2 nights per week. However, priority will be given to complex Continuing Care patients and those in the final stages of a terminal illness. We will liaise with Marie Curie for any extra assistance and support that may be required.
Key Individuals
All relevant staff are based in Community Nursing, Lansdowne, Sanatorium Road, Canton.
The Support and Sitting Service Co-ordinator will review sitting requests against the criteria and allocate as the service demand allows.
