CELT is a locality based intermediate care service providing "virtual ward" multidisciplinary support to individuals requiring increased health and social care input in the community (including care homes) to maintain and enhance recovery from acute problems. CELT can also provide input to patients who are being discharged from hospital who need a similar level of enhanced support.
The team has close links with the core District Nursing Teams, the Acute Response Team (ART), the Elderly Care Assessment Service (ECAS), Clinical Case Managers, Chronic Disease Management Teams, Voluntary Agencies and Community Mental Health Teams.
One of the main aims of this service is to provide additional support to General Practitioners (including 'Out of Hours' GPs) who have patients who are failing in the community or in a care home setting and who would benefit from MDT input. Other Primary Care Practitioners (e.g. District Nurses/Clincal Case Managers, Specialist Nurses etc) may also refer to the service providing the patients GP is aware of the referral. The service will also accept referrals for patients from within the locality area who require support following discharge from secondary care.

CELT will provide a service to individuals who reside in the East of Cardiff. The areas covered are:

- St Mellons
- Tremorfa
- Trowbridge

Hours of Service
CELT is open from 8.30am till 4.30pm, Monday to Friday and will accept referrals during these hours (excluding Bank Holidays). Referrals received when the office is closed will be dealt with the next working day.
