Heart Failure Nursing Team

The Heart Failure Nursing Team is part of the wider team which aims to improve the quality of patient care for patients with heart failure. 

The service aims to ensure that:

  • patients are taking appropriate evidence based therapy and monitored accordingly 
  • patients are provided with education and support for both them and their families 
  • patients are given the appropriate knowledge and skills to prevent admission where possible 
  • patients receive co-ordinated care between primary/secondary/tertiary and palliative services 
  • patients undergoing Cardiac resynchronisation therapy are followed up ensuring that their device therapy is optimised 
  • patients with a structural inherited cardiac condition (ICC) are provided with evidence based care and therapy for their cardiac condition, receive appropriate education and support, are referred to Medical Genetics and receive appropriate genetic counselling (e.g. regarding mode inheritance and genetic testing - psycho/social implications)
  • there is facilitation and co-ordination of cascade screening for individuals 'at risk' of a structural ICC
  • patients have access to the ICC patient support group (held quarterly).

The nursing service provides nurse-led clinics, phone contact and home visiting. It also acts as a resource and provides education for a variety of health disciplines across primary and secondary care.
